UnitedHealth Group Case Manager RN - Various locations in Wisconsin in Racine, Wisconsin

We need a care coordinator in Wisconsin and we are open to where that person resides. This position can either cover Kenosha/Racine, Milwaukee, Green Bay or Wausau area. If you are located in or near any of these areas, please apply and know that the travel will be local to the area that you live.

Primary Responsibilities:

  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction

  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels

  • Assist members with the transition from a care facility back to their home

  • Occasionally visit members in their homes and/or other settings, including community centers, hospitals or providers' offices

This position will involve travel within the local area and the percentage of travel can vary week to week. Most of the time you’ll work autonomously, so the ability to stay focused and motivated is essential.

Required Qualifications:

  • Current, unrestricted RN license in the state of WI

  • 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management

  • Access to reliable transportation

  • Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications

Preferred Qualifications:

  • Case management experience including Certification in Case Management

  • Home care, field based case management or telephonic case management experience

  • Behavioral Health experience

  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs

  • Experience in Home & Community based or Long Term Care services delivery

  • Bilingual skills (Spanish)
