Job Summary:

The Practice Facilitator is responsible for working with CMS directives, health plan initiatives, systems and practices to facilitate practice redesign efforts, utilizing innovative thinking about how to best support achievement towards improved process and clinical outcomes, decreased costs, and increase in efficiency that results in quality measure attainment and provider and health care team satisfaction.


Bachelor's Degree or higher in a related field preferred, RN certification a plus.

Licensure, Registration and/or Certification:

A valid driver's license, Motor Vehicle Report, and proof of vehicle liability insurance in the amount required by SFHS guidleines will be required.

Work Experience:

5 - 7 years related experience.

Knowledge, Skills and Abilities:

Proficient knowledge of Microsoft Outlook, Access, Word, and Excel. Working knowledge of Epic Practice Management software and medical record documentation,documentation software, and concepts of quality reporting. Working knowledge of Health System and Medical Office policies and procedures. Effective interpersonal, written and oral communication skills. Strong presentation and delivery skills. Ability to prioritize work in an effective and efficient manner. Ability to problem solve and exhibit initiative in analysis and teamwork activities.
